Adolescents, Ages 12-15, Now Eligible to Receive Pfizer Vaccine

North Oaks Immunization Clinic is responding to the FDA’s recent expansion of emergency use of the Pfizer-BioNTech COVID-19 vaccine to include adolescents, ages 12-15, by scheduling several mass vaccination events. Appointments are required and can be scheduled here or by calling (985) 230-SHOT [7468].
Mass vaccination event dates and times include:
- Saturday, May 15, from 8 a.m.-12 p.m.
- Saturday, May 22, from 8 a.m.-5 p.m.
Individuals, ages 12 to adulthood, are eligible to receive the Pfizer-BioNTech COVID-19 two-dose vaccine series. Individuals, ages 18 and older, are eligible to receive the Johnson & Johnson (Janssen) one-dose COVID-19 vaccine. Either vaccine type may be scheduled May 15 and May 22.
For a person younger than age 18, parental/caregiver consent is needed to get the Pfizer vaccine. This consent form can be found on the Louisiana Department of Health website at www.covidvaccine.la.gov. A link to the form also is available at covidvaccine.northoaks.org.
The Immunization Clinic is located on the North Oaks Medical Center campus in Hammond. Those with appointments should enter the campus via Medical Arts Plaza Drive from Highway 51 Business/Veterans Boulevard, use Parking Lot C and follow the blue “IMM” signs to the Immunization Clinic entrance.
